    Chronic Care Nurse - Reimagined Primary Care

    Campbelltown, NSW

    About the role

       MyHealth, Amplar Health and Medibank have teamed up to launch an exciting new pilot program, designed by GPs, for patients, and supported by a diverse healthcare team. This innovative program focuses on improving patient outcomes in general practice through proactive care and multidisciplinary case management and care coordination to provide more timely and comprehensive care to our patients.     We are looking for a Chronic Care Nurse to join us on a full-time contract basis (completion July 2026). This is an excellent opportunity for an organised nurse looking to transition out of bedside care to a multidisciplinary leadership role. This successful candidate will be responsible for coordinating patient appointments with clinicians and collaborating with GPs and others to provide chronic disease management. The role will be based at our Campbelltown practice, with at least one day working from home.    A day in the life:
    • Collaborate directly with GPs and other healthcare providers including social workers and nurses to provide high-quality patient care and improve patient outcomes
    • Review and segment patient appointments to GPs and allied health providers based on defined processes and protocols 
    • Conduct clinical assessments such as Health Assessments to identify preventative and proactive health actions and to ensure comprehensive care for each patient
    • Prepare and provide input into care planning processes for chronic diseases, including Care Plans
    • Deliver care coordination to support patients to achieve their health and wellbeing goals and enable integrated care
    • Provide health promotion and education to support health literacy and self-management
    • Monitor and manage capacity of the clinics to reduce patient waiting times and enhance access Organise and manage the treatment room to ensure it runs smoothly and efficiently
    • Assist in medical procedures and provide support where necessary
    • Manage wound care and administer immunisations and injections to support patients health needs
    • Perform additional clinical tasks such as Spirometry, ECGs, and ABIs as needed
    What we're looking for:
    • AHPRA Registration in Registered Nursing
    • Minimum 3 years of experience in general practice
    • Demonstrated ability to coordinate and designate patient flow in a fast paced environment 
    • Experience in providing chronic disease management in primary care settings.
    • Strong communication and interpersonal skills including the ability to influence and facilitate change
    • The ability and the enthusiasm to work independently and as part of a team
    • Strong multitasking abilities and effective prioritisation of workflow.
    • Experience with Best Practice software
    • Experience with CareMonitor desirable
