    Social Worker - Reimagined Primary Care

    About the Role

    MyHealth, Amplar Health and Medibank have teamed up to launch an exciting new pilot Primary Care Project, designed by GPs, for patients, and supported by a diverse healthcare team. This innovative program focuses on improving patient outcomes in general practice through proactive care and multidisciplinary case management and care coordination to provide more timely and comprehensive care to our patients. 

    We are seeking a friendly & professional Social Worker to join us on a full-time contract basis (completion July 2026). Recognising health is determined by a wide range of factors and issues, our model has a focus on identifying and supporting patients with social wellbeing needs. We are running the pilot in our practices across Parramatta, Auburn, and Campbelltown. This is a flexible position that offers a mix of in-clinic and hybrid working options, depending on patient demand and administrative workload.

    In this role, you will:

    • Participate in case conferences and collaborate with GPs, nurses, and allied health providers to contribute to care planning and team-based care
    • Conduct social wellbeing assessments and liaise with social service and community agencies
    • Provide referrals to relevant social and community services and offer service navigation to patients and families.
    • Support patients in self-management of their health and wellbeing, providing guidance and resources.
    • Provide care coordination and case management, including monitoring patient status, recalling patients, and coordinating follow-up appointments.
    • Offer advocacy support and individual/family counselling for patients, families, and carers.
    • Provide crisis intervention, safeguarding of vulnerable patients and other evidence based therapeutic interventions, as needed 
    • Proactively identify and maintain professional relationships with community and social services available in local areas.
    • Provide education to other clinicians on how to manage vulnerable patient situations.
    • Participate in project monitoring and evaluation activities to drive continuous improvement and inform the independent evaluation of the project.

    What we are looking for

    • Current understanding of social services (both in Western Sydney and Nationally such as ACAT)
    • Minimum 2 years experience as a Social Worker in a health setting in Australia
    • Ability to work independently and as part of a team
    • Understanding (and preferred experience) with health coaching
    • Work independently and as a collaborative member of the multidisciplinary team
    • Strong communication and interpersonal skills including the ability to influence and facilitate change
    • Hold Australian Citizenship/Permanent Residency or Valid Work Visa
    • Able to prioritise competing demands
